Lucēre Vision respects your privacy. This policy explains how information may be collected and used through our website and connected services. It does not replace a HIPAA Notice of Privacy Practices, which addresses patient health information and medical records.

When you contact us, we may receive your name, contact details, and information you choose to provide. When connected appointment or patient-service tools are available, they may also collect information needed for scheduling, communications, care, or payment.

Information may be used to respond to inquiries, support requested services, operate and protect the website, and meet legal obligations. Embedded services may receive technical information when their content loads, and their own privacy notices may also apply. Listing a provider here does not authorize it to receive medical records.
Patient information and medical records
Patient health information is subject to applicable health-privacy laws, including HIPAA where applicable. Uses and disclosures for treatment, payment, health care operations, or other purposes must meet the relevant legal requirements. Where a separate authorization is required, browsing this website or accepting cookies does not provide that authorization.
Please do not send medical records, diagnoses, insurance details, or other sensitive health information through general website forms or ordinary email. Contact the practice for an appropriate secure method. Website messages are not for emergencies.
Where HIPAA applies, patients have rights including access to their records, requests for amendments, certain communication and disclosure restrictions, and privacy complaints.
